Petition for the First Degree in Masonry

Basically, this is an application (petition) to join a Masonic Lodge. It all starts here!

This form must be printed and completed then submitted physically to the lodge by USMail or in person.

Application for a Certificate of Good Standing

Apply to receive a Certificate of Good Standing showing your current status with this lodge. For Members.

This form may be completed and submitted electronically throiugh email to the secretary.

Petition for Affiliation by Certificate of Good Standing

Form 39

Request membership with our lodge (Plural or by Transfer from another lodge) by presentation of a Certificate of Good Standing from your current parent lodge.

This form may be completed and submitted electronically throiugh email to the secretary.

Petition for Reinstatement

This form must be submitted by suspended members to have thier membership with this lodge reinstated. Please contact the secretary for necessary details.

This form may be submitted by email by scanning after signatures have been collected or by calling secretary.

Application and Agreement to Purchase Endowed Membership on Installment Purchase Plan
